This site is operated by Velocity Web Enterprises LLC d/b/a Check Disability (“Check Disability,” “we,” “us,” or “our”). We are a privately owned information and referral service and are not affiliated with or endorsed by the Social Security Administration (SSA) or any government agency.
2) Scope
This Privacy Policy applies to our websites, forms, and services that link to it (collectively, the “Services”). It does not cover third-party websites or services, including those of our participating advocates/attorneys (“Partners”). Their practices are governed by their own privacy policies.
3) Information we collect
Information you provide (examples):
- Contact details (name, phone, email, ZIP code, best time to reach you)
- Questionnaire responses (age range, work status, duration of condition, medical condition categories, prior applications)
- Consent preferences (calls/texts/emails), communications with us (including call recordings where permitted by law)

- Self-reported health or disability-related information used to help assess potential eligibility and to facilitate a referral at your request.
Information we collect automatically (via cookies, pixels, or similar):
- Device & usage data (browser type, pages viewed, time on page, referring/exit pages)
- Approximate location (derived from IP address)
- Ad/attribution data (e.g., UTM parameters, GCLID)
Information from partners & vendors (where applicable):
- Lead distribution platforms, call/SMS providers, analytics providers, advertising networks may provide metrics, status updates, or campaign performance data.
4) Sources of information
- Directly from you when you use the Services or communicate with us.
- Automatically via cookies and similar tools.
- Service providers who perform services on our behalf (e.g., hosting, analytics, call/SMS, lead management).
- Partners who inform us about referral outcomes or communications, as allowed.
5) How we use information
- Provide the Services, including generating an informational eligibility signal and facilitating referrals at your request.
- Communicate with you (phone, text, email) with your consent, including reminders and follow-ups. Consent is not a condition of purchase; message/data rates may apply.
- Operate, secure, troubleshoot, and improve the Services; develop new features.
- Measure and attribute marketing performance; detect, prevent, and respond to fraud or misuse.
- Comply with legal obligations and enforce our Terms of Service.
7) Advertising, analytics & cookies
We (and third parties) use cookies, pixels, and similar technologies to operate the site, measure performance, and, where allowed, improve or personalize ads.

Do Not Track: We do not currently respond to browser “DNT” signals.
8) Data retention
We retain information for as long as reasonably necessary to provide the Services, comply with legal obligations, resolve disputes, and enforce agreements. If you request deletion, we will also direct applicable service providers and (where feasible) Partners to delete your information as required by law.
9) Security
We use reasonable administrative, technical, and physical safeguards designed to protect your information. However, no method of transmission or storage is completely secure, and we cannot guarantee absolute security.
10) Your choices & rights
- Marketing emails: Use the unsubscribe link in any email.
- Text messages: Reply STOP to opt out; HELP for help. Message/data rates may apply.
- Calls: Ask our agent to update your contact preferences during any call.
- Access/Deletion/Correction: You may request access to, deletion of, or correction of your personal information by contacting us (see Contact us). We will verify your request and respond as required by law.
- Ad/analytics preferences: Adjust browser or platform settings and see State notices.
11) State notices (U.S.)
Depending on where you live, you may have additional rights:
- California (CCPA/CPRA): Right to know/access, correct, delete, and to opt out of sale or sharing of personal information (as defined by California law); right to limit use/disclosure of sensitive personal information; and non-discrimination. - Colorado, Connecticut, Utah, Virginia, and others: Similar rights to access, delete, correct, and opt out of targeted advertising, sale, or profiling. Use the same methods above.
- Nevada: Residents may opt out of the sale of covered information by emailing privacy@checkdisability.example.
Sensitive information: We only collect limited health-related details that you provide to help assess potential eligibility and facilitate referrals with your consent. We do not use sensitive data for inferential marketing.
12) EU/UK GDPR
If GDPR applies, our legal bases include: consent (e.g., for sharing with Partners, certain marketing), legitimate interests (e.g., site operation, fraud prevention, basic analytics), contract (to provide requested Services), and legal obligations.
EU/UK residents may have rights to access, rectify, erase, restrict or object to processing, portability, and lodge a complaint with a supervisory authority. To exercise rights, contact us (see below). If we rely on consent, you may withdraw it at any time.

13) Children’s privacy
The Services are intended for adults. We do not knowingly collect personal information from children under 13 (or under 16 where applicable). If you believe a child has provided personal information, contact us and we will take appropriate steps to delete it.
14) International data transfers
We may store and process information in the United States and other countries. Where required, we use appropriate safeguards (e.g., standard contractual clauses) for cross-border transfers.
15) Changes to this policy
We may update this Privacy Policy periodically. We will post the updated policy with a new effective date. Your continued use of the Services after changes become effective means you accept the updated policy.
